 Subject:     Request for Quotes (RFQ) for Janitorial Services

 Castro Valley Sanitary District (CVSan) is a small public agency located in Castro Valley, CA.  CVSan is requesting quotes for janitorial services for fiscal year February 1, 2012 thru January 31, 2013.

 The selected contractor will be required to enter into the District’s standard consulting agreement (template agreement attached).  Please note the following items to be finalized and incorporated into the agreement prior to signature:

CVSan is requesting that each bidder submit a quote.  Please see the attached Guideline for Service for a complete list of requirements.

 B.  Information about your company

• Contact Information
• Total number of current customers
• Total number of customers who are public agencies (city, county, state, special district)
• Total number of employees
• Number of years in business

 C.  List of Contacts for References, minimum of three (3)

Please attach a customer reference list, which includes contact name, contact title, company name and city, contact phone number, contact email address, and website address. 

Please email or fax quotes, attn: Pat Krevey, to pat@cvsan.org or (510) 537-1312 by Tuesday, January 3, 2012 by 2:00 p.m.  Quotes will be evaluated based on total project cost.  A final selection will be made by CVSan by Friday, January 13, 2012.

 A Questions and Answer period concerning this RFQ will be managed by Pat Krevey at pat@cvsan.oror designee.  The period closes on Wednesday, December 21, so please submit questions in writing accordingly.  Responses to all questions will be posted at www.cvsan.org“Who we are” >Request for Proposals” by 5:00 p.m. December 28, 2011.  The author of the questions will not be identified on the website.
